New Patient Forms Our patients are busy people and we do everything possible to minimize your waiting time. If you are a new patient to our practice, please print and fill out the New Patient Form and bring it with you.
Payment Options In general, payment is expected at the time of service, but we realize that may not always be convenient for our patients so we have 3 payment plans for you to choose from: 1. Interest-free Payment Plan (for up to 12-months) with no down payment necessary through Healthcare Finance. All paperwork must be processed before the procedures begin. Please contact our office for the paperwork at least 1 month in advance of your appointment. 2. Prepayment of dental services 72 hours prior to visit with either cash, check, Mastercard, Visa or Discover will receive a 10% courtesy discount. 3. Payment in full on the day of service will receive a 4% courtesy discount. Senior citizens who pay in full at the time service will receive an 8% discount.
